Service Description
Receive a certification of cannabis oil through our Nurse Practitioner (you will get full credentials of nurse practitioner). Applications are submitted to Virginia Board of Pharmacy and you will receive your medical cannabis card from Virginia Board of Pharmacy via mail. This service is recommended for experienced CBD users who are willing to try THC products or find that CBD is not working for them.
Documents Needed
- A copy of the written certification issued by a registered practitioner
- Proof of residency of the qualifying patient and proof of residency of a parent or legal guardian, if applicable, such as a government-issued identification card or tax receipt
- Proof of identity of the qualifying patient and, if the patient is a minor, proof of identity of the parent or legal guardian in the form of a government-issued identification card
- Proof of the qualifying patient’s age in the form of a birth certification or other government-issued identification
- A valid Virginia Driver’s License will fulfill the requirements for proof of age, identity, and residency
State Laws For Medical Marijuana in Virginia
- A Medical Professional or a Physician must be registered with the Virginia Department of Health Professional and the Pharmacy Board, respectively
- A Medical Health Professional must keep an updated registration with the Pharmacy Board
- Patients must be endow with state-issued identity proofs and a valid MMID card whenever asked
